Patient Registration
Please see below for instructions on how early to arrive prior to your scheduled appointment. It does differ for new verses established patients. In order to provide efficient care for our patients, you may be asked to reschedule your appointment if you do not arrive early. Bring the following:
- Current insurance card
- Valid government-issued photo ID
- Your completed registration or consent forms
- Copay
New Patient Paperwork
All the forms below are required for all new patients. In addition, we’ve also provided the Authorization to Release Health Information form for your convenience.
Important: Please arrive at least 30 minutes prior to your scheduled appointment. In order to provide efficient care for our patients, you may be asked to reschedule your appointment if you do not arrive at least 30 minutes early. Please bring all completed forms to your next appointment.

Established Patient Paperwork
Established patients are required to complete the Registration Agreement below if you’ve had any changes to your information. In order to reduce your wait time at check in, please go ahead and fill out your Registration Sheet and Registration Agreement (if anything has changed since your last visit), and bring the completed forms to your next appointment. Please arrive at least 15 minutes prior to your scheduled appointment.

Billing
We will be happy to file your insurance for you; however, it is important that you provide us with the correct insurance information. Any change in your name, address, or insurance plan should be given to our front office staff upon your arrival to our office.

Prescription Refills
We don’t accept medication refill requests from pharmacies. If you need a prescription refill, please contact our office with your request.
